If you get a (fake) facebook notification from me, I’m extremely sorry. I was just clicking around the billion or so invites and notifications that build up and got careless. I don’t use facebook all that often, especially lately, but usually I spot fakes like this from miles away through a snowstorm.

See that top one? That’s the culprit here. Note the icon.
And now compare that to the official facebook video icon.

Allowing this app to have access to your profile to see what phantom video you’ve been tagged in will automatically send out notifications to a chunk, if not all, of your friends on your list and direct you to a page full of pointless apps. I removed the app instantly but the damage was already done :(
Remove the Fake Application:
I got switched to the new layout the other day so I do not know if these instructions are the same for the older one. Click on Account, top right, then Application Settings. Scroll down to the fake video (or whatever else you’d like to remove) and click on the X on the right end of the row. Ugh. I hope this thing gets curbstomped quickly!

I hate Facebook. But I have a lot of family and friends on there who make use of it, so I haven’t attempted to remove myself yet. I have completely locked everything down though.
I wish there was an option somewhere to remove ALL applications, current ones and any in the future. The way the apps work on Facebook are annoying and devious. You give away too much privacy whenever you say “accept”. I hate the way they clutter up my feed as my friends start using these annoying apps.
